Dear Parents, your child will be given a parental COVID vaccine consent form today. IF you wish for your child to receive the COVID vaccine at school by Copper Queen Medical Associates, please return it by Monday, 12/20/21. We are not mandating or promoting the vaccine, but rather providing a venue. Thank you!
